GEORGE PLETTS MERITORIOUS SERVICE AWARD

CRITERIA:

The Nominee has demonstrated an exceptional professional standard in their employment. This standard of achievement must be verified by their Supervisor.

The Nominee has provided service to their community by volunteering with civic organizations: such as the Boy Scouts, Girl Scouts, youth or senior adult programs, religious or similar organizations.

Provides an example of commitment and professionalism within the CSDIAI through past and present service.

May include, but are not limited to, active service with local and state professional organizations.

Must be a member in good standing with CSDIAI.

NOMINATION PROCESS:

Any member in good standing may nominate Active or Life Members in good standing, for the award to the Resolutions Committee.

Nominations are to be submitted to the Resolutions Committee Chairperson on a formal worksheet along with a resume of qualifications. A request for nominations will be published in the CSDIAI Digest for at least three (3) months prior to the closing of such nominations.

DEADLINE FOR SUBMITTING APPLICATIONS IS:  DECEMBER 31, 2010
